The
Provider would be working 8:30am-5:00pm Mon-Friday with great

base salary from $80,000-$95,000 (Depending Upon Experience) and a full

Program. The
HFNP - led visits are to ensure appropriate patient transitions from

the signs and symptoms of worsening disease. Position has an
expanded

quality initiatives, improving patient satisfaction and reducing
readmission rates. Must have 3-5 years experience
as a Nurse
Practitioner with cardiovascular patient care experience. Licensed in
the state of Arizona. Completion of the minimum required for
State or
National Certification with certification in the NP's area of
specialization (adult, family, etc) as required by the Arizona Board of

The Heart Failure Nurse Practitioner
(HFNP) will practice in the inpatient and some outpatient settings. The
HFNP obtains health histories and provides complete physical
examinations on patients recently discharged from the inpatient setting
with a primary diagnosis of heart failure. The HFNP will request and
interpret laboratory results, X-rays; prescribe and manage medications
and other therapies; provide healthcare teaching and supportive patient
counseling patients with an emphasis on prevention of illness and
health maintenance; consult and refer patients to other healthcare
professionals as needed. The HFNP will coordinate patient
post-discharge visits with the appropriate treating
physician. The
purpose of these HFNP-led visits is to ensure appropriate patient
transitions from the inpatient to outpatient setting, to include
verification that the patient has obtained medications, understand
medications, is taking medications and understands the diagnosis of
heart failure, including the signs and symptoms of worsening disease.
The HFNP will work closely with colleagues to ensure integration of
services and continuity of care for heart failure patients. The HFNP
will play a pivotal role on the Heart Failure program, working with

patient satisfaction and reducing readmission rates. In compliance with
the scope of a Nurse Practitioner practice, the HFNP acts as a care
provider, educator and communicator. While using critical thinking
skills, good judgment and strong organizational skills, the HFNP is
comfortable in a fast-paced and sometimes stressful critical care
environment. Occasional call and/or work on weekends may be necessary.
Performs
under the supervision of the Medical Director of Heart Failure Program
that is currently an active or emeritus member of the Department of
Medicine.
Education Required: Licensed in the state of Arizona.
Completion of the minimum required for State or National Certification
with certification in the NP's area of specialization (adult, family,
etc) as required by the Arizona Board of Nursing. Able to meet
requirements to be approved and credentialed by the YRMC Medical Staff
board for privileges and ability to shadow/observe during the
credentialing period must meet Medical Staff Guidelines. BCLS and ACLS
certifications required.
Experience Required: 3-5 years
experience as a Nurse Practitioner with cardiovascular patient care
experience. Excellent verbal and written communication skills. Computer
proficiency (must be able to pass computer proficiency assessment)
Experience
Preferred: Nurse Practitioner with heart failure experience preferred.
Bilingual in English/Spanish. Experience with EPIC Electronic Health
Record or similar electronic record systems.
